Dental Records: We keep detailed dental records including drug sensitivities, diagnostic tests, examination history, treatment, advice, referrals, communication with laboratories and other practitioners, x-rays, photos, etc. to help us provide appropriate ongoing care. We may obtain this information from other practitioners or share this information with other practitioners as part of this ongoing care. Personal details are verified regularly, and treatment records are updated at every consultation.

    Administrative Data: Our practice collects administrative data for accounting purposes, including your name, phone number, contact address/billing address, private healthcare fund and number, Medicare number, financial records of accounts and payments, insurance claim records, and work-related injuries.

      How is Your Information Used?

      1. The information collected will be used for the purpose of providing treatment to you. Personal information such as your name, address, and health insurance details will be used for communicating with you about our services and any issues affecting your treatment, including accounting purposes.

All your dental records will be de-identified before use for any of these purposes. Your personal identity will not be disclosed without your consent.
